Taking Care of Your Nose Bleeding

  • Ocean nasal spray (saline or salt water) 2 sprays each nostril 5 X daily for at least 1 month. It is extremely important to keep your nasal passage as moist as possible, and it would be preferable for you to use the spray more than 5 X daily, particularly during the winter months when the air is most dry from heating of indoor spaces.

  • Afrin nasal spray 2 sprays each nostril each nostril 2 X daily for 3 days. This can also be used during an episode of nose bleeding as frequently as needed.

  • Place a large amount of bactroban or bacitracing ointment around the opening of your nostrils and slightly inside the nostrils at least 2 X daily, again more often is preferable.

  • A humidification system (humidifier) should be placed at the patient’s bedside to maintain optimal humidity while sleeping and resting.

  • The patient should sleep/rest elevated > 30 degrees to decrease the pressure on the small blood vessels of the nose.
  • If you have high blood pressure, this must be controlled optimally with a systolic blood pressure less than 130 (BP < 130/x).

  • Never blow your nose; always sneeze and cough with your mouth wide open; and never place anything in your nose unless directed to do so.

  • If you are taking aspirin, warfarin (coumadin), plavix, or another anticoagulant, you will need to discontinue the medication for the next week. However, please check with your primary care provider or whoever prescribed the medication before discontinuing it.

IF YOUR NOSE BLEEDS AGAIN:

Place 3-4 large squirts of afrin in each nasal passage, and apply significant pressure to the nasal alae for at least 20 minutes while watching the clock. Release pressure after 20 minutes and repeat the process using copious amounts of afrin if the nose is still bleeding. If the nose continues to bleed after 2 X 20 minute pressure attempts, go to the ED or call our office. Attempt to control your anxiety and high blood pressure, and placing an ice pack on the nose can help slow bleeding.
